The undersigned hereby grant(s) Maple Craft USA permission to contact and obtain credit information from all references supplied as well as from the Credit Bureaus or other Credit Reference Services. The undersigned agree(s) to all credit terms of Maple Craft USA including specifically the following terms:

TERMS AND CONDITIONS

In addition to the terms and conditions set forth in any subsequently executed agreement or similar document, all purchases made by Applicant shall be subject to the following terms and conditions:

(1) Maple Craft USA reserves complete discretion concerning all extensions of credit and shall have the right at any time to refuse to extend credit to Applicant.

(2) Applicant acknowledges and agrees that all purchases made by it shall be paid for in accordance with such payment terms as may from time to time be established by Maple Craft USA. Until otherwise notified by Maple Craft USA, the following terms shall apply: (1) 2% 10 days, Net 30 (there are exceptions on large volume orders). (2) The net amount of each invoice is due 30 days from invoice date. Invoices are issued upon shipment initiation. (3) Cash discounts will be allowed if payment is received within 10 days from the invoice date.

(3) In the event that Applicant's account is referred to an attorney for collection, Applicant agrees that Maple Craft USA shall be entitled to collect, in addition to the principal amount, an attorneys fee of 15% thereof. This credit Application and any subsequent contract of sale between Maple Craft USA and Applicant shall be deemed to have been entered into at the home office of Maple Craft USA, in Albion, Pennsylvania and the Applicant and the undersigned hereby consent to the exercise of jurisdiction by the courts of the Commonwealth of Pennsylvania and further agree the venue shall lie exclusively in the Court of Common Pleas of Erie County in the event that any suit is instituted by Maple Craft USA, in connection with any cause of actions arising from or relating to this Credit Application or any extension of credit hereunder:
